The 24-hour Crisis Line is a program of Crisis Support Services of Alameda County. Trained crisis intervention counselors are available to receive crisis calls and give supportive counseling 24 hours a day, every day. Translation is available in more than 140 languages. (CSS) began providing In-Home Counseling services to elderly and frail home-bound older adults in 1993 with a small grant from the Area Agency on Aging. Since 2005, the program has been expanded to provide counseling services to older adults at our agency. Transitional Mental Health Services
The Aftercare program offers low cost options to persons who have completed at least 8 weeks of inpatient treatment. The aftercare program includes stellar housing with allowances for food. Citywide Linkage Team provides short-term, assertive, clinical case management, both in clinic and via outreach. Engagement of the client begins before s/he leaves the hospital and clients are followed until they are actually linked to ongoing mental health services. To foster the stabilization process, CLT attempts to link clients to housing services, financial entitlements (GA, SSI), MediCal, etc., though often these referrals can only be initiated during our brief treatment period.
