Chronic Care Program
MNHC is committed to improving the lives of individuals with chronic conditions. We provide health education to improve patient confidence to manage their conditions, helping them to avoid complications, build greater self-efficacy and improve their health. Patients may participate in individual self-management instruction or group health education. Programs include individual one-on-one counseling, monthly group diabetes education series, a peer to peer diabetes program, and weight management support.
As part of its comprehensive prenatal program, MNHC offers women the opportunity to prepare for their pregnancy in a group setting that fosters learning and community support. Groups of typically 8-10 women who are due to deliver in the same month meet together with a facilitator (midwife, social worker or health educator) throughout their pregnancy and after delivery.
