Program Details

Eligibility:

Seniors 65 and over, adults with a disability, and adults dependent on caregivers. Income is not a factor for eligibility.

Application Process:

Anyone may submit a confidential report. A person can also self-report if they are experiencing abuse of any form.

Documents Required:

There is paperwork involved in filing a report, along with the phone call. If you are employed by a financial institution, please complete form SOC 342. All other persons should complete form SOC 341.The forms are called:Report of Suspected Dependent Adult/Elder Abuse, SOC 341Report of Suspected Dependent Adult/Elder Financial Abuse, SOC 342 Spanish translations available.
