Citywide Linkage Team

Citywide Linkage Team provides short-term, assertive, clinical case management, both in clinic and via outreach. Engagement of the client begins before s/he leaves the hospital and clients are followed until they are actually linked to ongoing mental health services. To foster the stabilization process, CLT attempts to link clients to housing services, financial entitlements (GA, SSI), MediCal, etc., though often these referrals can only be initiated during our brief treatment period.

Program Details

Eligibility:

Client are typically referred to CLT when the PES or Inpatient Psychiatric Treatment Team at SFGH feels the client is unlikely to be able to remain stable in the community, or able to access necessary support and appropriate resources without CLT intensive assistance. CLT provides services to clients between the ages of 18 and 59 with chronic or acute mental illness and substance use issues, often with medical co-morbidity, and with an array of psychosocial stressors, including poverty and homelessness. Older clients are considered on a case-by-case basis
